Initial Evaluation

  • Q: What do I do prior to my initial appointment?

    A: Make an appointment online with our team, sign into your portal and fill out the necessary paperwork prior to your initial appointment.

    Q: Do you take insurance?

    A: We are in-network with:

    United Healthcare, Oscar, Oxford (including Optum, UMR, Golden Rule, Optum Behavioral Health, US Health Group, and GEHA)

    Aetna (including Meritain, Coventry, Nippon Life Benefits, Chapman Boone, and GEHA)

    Cigna (including Evernorth)

    Blue Cross and Blue Shields of NC - pending

    If your insurance is not listed, you are considered out-of-network. You will be responsible for the full amount at your visit and may then submit an out-of-network claim to your insurer. We are also able to provide you with a superbill per your choosing.

    We currently use a third party company to do our insurance verifications and billing, as this enables us to be able to provide exceptional service and more time with you. We will provide you with more information once you are an active client with us.

    Please contact your insurer regarding your out-of-network benefits, deductible, and co-pay.

    Q: What forms of payment do you accept?

    A: Payment in full is due at time of service. You may pay with cash, check, credit card, or your HSA card. You may leave a confidential credit card on file to simplify payment and billing.

    Q: Do you charge for cancellations?

    A: Cancellations and rescheduling within 24 business hours of your appointment will incur a $95 charge. Cancellations and rescheduling greater than 24 business hours notice will incur no charge.

Medication Management Follow up (I)

  • Q: Who do I contact for medication refills?

    A: Please check your bottle to see if you have any remaining refills prior to contacting us. You can also call the pharmacy and have them send a request to us electronically. Our fax number is (919) 910-9967.

    Additionally, you can contact us via your patient portal, call us at (919) 873-4339 and leave a message with your name, date of birth and medication/dosage request, or email us at admin@banyanwellness.org.

    Q: The pharmacy is not filling my medication and alerted me of a prior authorization (PA), what are my next steps?

    A: A PA is an additional verification step required by insurance companies prior to covering your medications. Have the pharmacy fax us the PA at (919) 910-9967 or get in touch with us regarding the PA so we can take care of it for you. Insurance companies take anywhere from 5-10 business days to approve or deny a PA request.

  • Q: How often do I have to see my provider for refills?

    A: Your provider will refill scripts every month for controlled substances and every three months for non-controlled medications once stable. This can change on a case-case basis. Initially you will be seen more frequently until therapeutic dosing is achieved.
